PERSONAL DOCUMENTS
• Italy Tourist Visa Application Form at the latest form
• Valid original passport (at least 6 months) + Old passport (if any).
• 2 photos size 4 x 6cm, white background, face seen clearly, less than 6 months
• Certified copy of all Household Registration Book’s pages.
• Certified copy of ID Card and curriculum vitae.
• Original and certified copy of birth certificate.
• Certificate of marital status (single, marriage, divorce).
•  Student ID card or certificate of having studied at the school affixed seal and of Board of Trustees.

EMPLOYMENT DOCUMENTS
If you are an employee
•    Labor contract / Appointment decision (certified copy of company).
•    Health Insurance.
•    Payroll for the least 3 months.
•    Original Leave of absence letter affixed company’s seal (reason and duration of travel to Italy).
If you are business owner
•    Business Registration Certificate.
•    Receipt of tax payment in the last 3 months (In case of online tax payment, please print it out and affix the company seal).
If you are a retiree
•    Retirement Certificate and pension book certified by local authorities.

PROOF OF FUNDS
•    Original Passbook and Bank Account Balance Confirmation Letter (valued at least USD 5,000 and submitted for at least 3 months up to the submission date).
•    Certificate of land use right, ownership of house, cars, etc. (if any).

TRAVEL ITINERARY
•    Travel schedule to Italy in details (place of visit, arrival time, return time…)
•    Booking confirmation of round trip flight ticket to Italy and Hotel Booking Confirmation in Italy.
•   Italy travel insurance with a minimum value of EUR 30,000 for the entire length of stay in Italy and the Schengen area.

•    Receipt of corporate income tax payment in the last 3 months.
•    Bank Account Balance Confirmation Letter of Company.

Documents of the guarantee company
•    Original invitation letter affixed Company seal (specify details of contents, reasons, time of invitation, commitment to return home on time ; full name of the inviting company, name of the inviting company’s representative, company address and contact phone number).
•    Letter of guarantee affixed seal of company.
•    Business Registration Certificate
•    Receipt of Tax Payment.
•    Company’s seal specimen certificate issued within 3 months near the submission date.
•    Photocopy the passport page of the inviting person (whose name is in the business registration certificate)
•    Work schedule.

PROOF OF FUNDS
